Cheap health insurance: lowering your premium in 2026

Deductible, care model, insurer comparison, cancellation before 30 November: four quantified levers to reduce your LAMal basic premium in 2026.

GoLamal - Cheap health insurance: lowering your premium in 2026

Why does the basic premium vary so much from one canton to another?

Around CHF 1'500 per year: this is the typical gap between a basic premium in Basel-Stadt and in Appenzell Innerrhoden for an adult with a CHF 300 deductible. These amounts are orders of magnitude to be verified on priminfo.admin.ch for 2026.
The reason is simple: the premium for compulsory health insurance (AOS, assurance obligatoire des soins) reflects the healthcare costs of the canton. The density of doctors, hospital provision, the age of the population and the use of care vary from one region to another. Each canton therefore has its own average cantonal premium, set on the basis of its actual costs.
Within a canton, premiums also differ by premium region (up to three per canton) and by insurer. For the same statutory coverage, the gap between the most expensive and the cheapest offer in a region can reach several hundred CHF per year.
Premiums are approved each year by the Federal Office of Public Health (FOPH) and then published in the autumn, generally at the end of September. They should be compared on the official comparison tool, which presents up-to-date rates for each municipality.
A useful point of terminology: the law (LAMal) imposes the same catalogue of benefits on all insurers. Only the premium, the insurance model and the service differ. Basic insurance is not to be confused with supplementary insurance, governed by the LCA (Insurance Contract Act), where insurers freely set their own conditions.

Optional deductible: from CHF 300 to CHF 2'500, at what point does?

Moving from a CHF 300 deductible to CHF 2'500 reduces the premium by around CHF 1'500 per year for an adult: worthwhile only if medical expenses stay below approximately CHF 2'000. An order of magnitude to be verified according to canton and insurer.
The calculation is as follows. With a CHF 2'500 deductible, you pay CHF 2'200 more out of your own pocket before the insurer steps in (CHF 2'500 minus CHF 300). If the premium saving is CHF 1'500, the break-even point is at CHF 700 of additional expenses. Beyond that, the high deductible costs more than it saves.
The optional deductibles for adults are CHF 300 (the ordinary deductible), CHF 500, CHF 1'000, CHF 1'500, CHF 2'000 and CHF 2'500. For children, they range from CHF 0 to CHF 600. The premium reductions by step are capped by law.
In addition there is the co-payment: 10% of costs above the deductible, capped at CHF 700 per year for an adult (CHF 350 for a child). The maximum annual burden with a CHF 2'500 deductible therefore reaches CHF 3'200 (CHF 2'500 + CHF 700).
The real pitfall: a high deductible is advantageous in good health, but weighs heavily in the event of unforeseen expenses (accident, hospitalisation, treatment). You need a reserve of at least CHF 3'200 to bear this without difficulty.
Moving to a lower deductible is possible only on 1 January, with notice given before 30 November. Moving to a higher deductible is possible at any time for the following year, subject to the same deadline. Source: LAMal, art. 62 and OAMal, art. 103.

Family doctor, HMO, Telmed models: what premium reduction, what?

Up to 10 to 15% lower premiums: this is the usual range for alternative insurance models compared with the standard model, according to the FOPH. The exact figure depends on the insurer and the canton, to be verified on priminfo.admin.ch.
Three models dominate in Switzerland.
  • Family doctor: you first consult a doctor chosen from an insurer's list, who refers you to specialists if necessary.
  • HMO: you consult a group health centre, which coordinates all of your care.
  • Telmed: you call a telephone medical advice service before any consultation.
The common constraint is the restriction on free choice of doctor. In return, the insurer reduces the premium because the care pathway is coordinated. Exceptions exist in all models: emergencies, gynaecology, paediatrics and ophthalmology are generally accessible without prior referral, but the rules vary according to the insurer.
One point to check before switching: the list of partner family doctors in your region. If your current doctor is not on it, the premium saving is paid for by changing practitioner.
If you leave the alternative model, the insurer may refuse to readmit you at the previous rate; the conditions for leaving are set out in the insurance regulations. For chronic health situations involving several specialists, the standard model retains an advantage in flexibility.

Comparing insurers (Assura, Swica, Groupe Mutuel, etc.): which?

Zero francs of difference in benefits: basic insurance guarantees the same catalogue to all insurers, under the LAMal. The differences lie in the monthly premium, the models offered, the service and the reimbursement time.
Assura, Swica, Groupe Mutuel, CSS, Helsana, Sanitas and other insurers are cited here as examples for comparison, without any ranking. Their premiums change every year: only the official FOPH comparison tool (priminfo.admin.ch) displays up-to-date amounts for your municipality, your age and your deductible.
Beyond the premium, a few criteria are worth checking:
  • The alternative models offered (family doctor, HMO, Telmed) and their reduction in CHF.
  • Contact channels: online customer area (such as the Assura login), telephone, local agency, response time.
  • Speed of reimbursement of bills, if you advance the costs.
  • The handling of supplementary insurance: a supplementary policy (LCA) may be refused or modified after a change of basic insurer, independently of the AOS.
An important warning: changing insurer for basic insurance does not oblige you to change supplementary insurance, but an insurer may link the two in bundled offers. Supplementary policies are subject to a health questionnaire, unlike basic insurance, where admission is compulsory and without reservation.
Private comparison sites (Comparis, FinanceScout24, caisse-maladie.ch, Helvicare) offer practical tools, but some receive a commission if a policy is taken out. The Confederation's comparison tool remains neutral.

How to cancel before 30 November and not miss the deadline?

30 November: the cancellation of basic insurance must reach the insurer by that date at the latest, by registered post. Sending it on 30 November is not enough: it is receipt that counts, under the LAMal (art. 7).
In practice, post the letter no later than the week of 20 November. Registered post generally takes one to two working days, but delays are not uncommon at the end of the month.
The letter must contain: your name and address, your insured number, a clear statement of cancellation of the basic insurance as of 31 December, and your signature. Keep the post office receipt as proof.
If the insurer has announced a premium increase, an extraordinary right of cancellation applies: you can cancel until the end of November for 31 December, or according to the deadline specified in the notice. The insurer is required to inform you of this right at least two months before the new premium takes effect.
An important step: only cancel after you have received confirmation of membership from the new insurer. If the new insurer does not acknowledge receipt, you risk finding yourself without cover. An insurer is obliged to accept anyone for basic insurance, but the membership procedure still has to be completed.
It is possible to change insurer on 1 July in the case of an alternative model or the standard deductible (notice by 31 March), according to the conditions of your current insurer. Check your contract. In all cases, the obligation to be insured in Switzerland continues without interruption.

Co-payment, cantonal subsidies, premium reduction: what remains to be

CHF 700 per year at most: this is an adult's annual co-payment (CHF 350 for a child), in addition to the deductible. It represents 10% of costs above the deductible, under the OAMal.
For an adult with a CHF 300 deductible, the maximum annual burden is therefore CHF 1'000 (CHF 300 + CHF 700). With a CHF 2'500 deductible, it rises to CHF 3'200.
The individual premium reduction (RIP, réduction individuelle des primes) helps people on modest incomes. Each canton defines its own criteria and amounts. According to the FOPH, around a quarter of insured persons in Switzerland benefit from it. The conditions depend on the relevant income, the composition of the household and sometimes on wealth.
The application is made to the competent cantonal body, often automatically on the basis of the tax assessment, but not everywhere. Depending on the canton, this is the cantonal health insurance office, the social insurance service or a compensation fund. In Valais, for example, it is the compensation fund that handles these applications. In Geneva, the health insurance service manages the subsidies.
Do not wait for notification: if you think you are entitled to a subsidy, submit the application within the deadline set by your canton, as registration is not always retroactive.
People receiving social assistance or supplementary benefits have their basic premium covered within the limits of the average cantonal premium. This makes it useful to choose an insurer whose premium does not exceed that ceiling, in order to avoid an out-of-pocket shortfall.

CHF 2'500 deductible: you save CHF 1'500 in premium, but bear up to CHF 3'200 in costs per year

Key takeaway

With a CHF 2'500 deductible, the maximum annual burden reaches CHF 3'200 compared with CHF 1'000 with CHF 300: the premium saving of around CHF 1'500 is worthwhile only if your additional costs stay below approximately CHF 700, hence the importance of a reserve of at least CHF 3'200.

Switch insurer or stay: a three-step calculation to do yourself

Three steps, a quarter of an hour: that is the time needed to compare your situation. Here is the method.
Step 1: note your current premium for 2026 (announced in the autumn) and your actual medical expenses for the past year, deductible and co-payment included. They appear on your statements.
Step 2: on priminfo.admin.ch, enter your municipality, your age, your deductible and your model. Note the annual premium of several insurers, plus your own. Calculate the gap in CHF per year. The amounts cited in this article are orders of magnitude: only the comparison tool gives the exact figures.
Step 3: test two scenarios. In the first scenario, your expenses stay close to those of last year. In the second scenario, an expensive unforeseen event takes you to the ceiling (deductible + CHF 700). A premium saving makes sense only if it remains greater than the risk assumed in the second case.
Worked example: an adult saves CHF 1'500 by moving from a CHF 300 deductible to CHF 2'500. If they had CHF 600 of expenses last year, they gain around CHF 1'300. If they had CHF 4'000 of expenses, they lose around CHF 400 compared with their initial situation, depending on the split between deductible and co-payment. Result: the high deductible suits mainly someone who consults rarely.
Also consider the costs of switching: a new care model, a new insurer to contact, supplementary policies to reconsider. If the gap is less than CHF 100 per year, the gain does not always justify the effort. Beyond CHF 300 to CHF 400, the calculation often favours switching.
